_POSITION SUMMARY:_

Under the direction of a licensed nurse or therapist, participates in the client's care and experience throughteamwork, effective communication and basic care skills.


Principal Accountabilities*

Community: Promotes a positive relationship between Sutter Health and the community.

Finance: Utilizes resources to deliver quality care in a safe, efficient, and cost-effective manner.

Growth: Follows client plan of care during the duration of client's enrollment in the program in order to achieveoptimal care and quality of daily living.

People: Participates as a member of the interdisciplinary health care team in order to foster collaboration forthe development of an effective client plan of care.

Quality: Provides optimal personal and restorative care to clients per Sutter Standards of Excellence toachieve the best possible outcomes and quality of daily living. Reports changes of clients conditions.

Documents care provided appropriately.

Service: Promotes client satisfaction with services through utilization of the Sutter Difference standards ofbehavior.

MINIMUM POSITION REQUIREMENTS:

Education:

Current California certification as a Certified Home Health Aide. Must be CPR certified on hire. For SutterSenior Care position, must acquire first aid and CPR Certification within six months of hire and maintaincurrent certification thereafter. Must possess a current driver's license, reliable transportation and proof ofliability insurance.

High School Graduate or equivalent.

Knowledge:

Medical terminology, healthcare practices and universal precautions.

Experience:

One year experience working with the elderly is required. One or more years of experience as a Home HealthAide, preferable caring for the elderly is preferred.

Skills:

Under the direction of licensed nurses or therapists, manages multiple demands in an ever-changingenvironment. Good organizational skills a must. Requires the ability to prioritize duties and have good timemanagement skills. Must have effective English, written and verbal communications skills. Bilingual skillspreferred. Sincere interest in working with frail elderly participants is a must. Ability to work as part of theinterdisciplinary team and make meaningful contributions to the development of the plan of care.
